Privacy Policy: In order to ensure the safety of your child, we need to collect some basic information including, but not limited to, current contact information, medical conditions and allergies. We understand the sensitive nature of this information, and every effort will be made to ensure its complete confidentiality. This information will be viewed by our staff (paid and volunteer), and shared within our organization (THRIVE) and our parent organization (LakePoint Family Church). If necessary, this information will also be shared with licensed medical professionals, including (but not limited to) nurses, paramedics, and doctors. Under no circumstances will this information be shared with a third-party without your consent, unless required by law. By signing this form, you acknowledge that you have read these terms and give consent for the information collected to be used in accordance with this privacy policy.
Expectations: This is going to be a great learning experience, and we want to keep it fun for everyone. This is the list of expectations we require of every student, no exceptions. Failure to comply may result in parents being called to come pick up their child.
1) Children must respect and obey our leadership staff.
2) Parents/Guardians must check-in their child(ren) with the leadership staff at the time of arrival.
Consent: I have read these expectations to my child(ren) or have explained them to my child(ren), he/she/they understand list of expectations and choose to abide by them. I understand that failure to comply may result in disciplinary action, including (but not limited to) limited participation and / or not being allowed to continue in the class.
Legal Stuff: In addition to the specific expectations outlined above, we also expect each student to conform to the basic rules of conduct established for all LakePoint Family Church / THRIVE events: No possession or use of alcohol, drugs, or tobacco; No fighting, weapons, fireworks, lighters, or explosives; Voluntary participation; Respect for property, one another, staff, and adult leaders; Compliance with event schedules. Students who fail to comply with these expectations may be asked to refrain from returning. By signing this release, you acknowledge that you understand the inherent risks involved, and hereby release LakePoint Family Church, and its pastors, employees, volunteer workers, from any and all liability for any injury, loss, or damage to person or property that may occur during the course of your child's involvement in this event. In the event that your child / dependant is injured and requires the attention of a doctor, you consent to any reasonable medical treatment as deemed necessary by a licensed physician. In the event treatment is required from a physician and/or hospital personnel chosen by the Church, or the hosting venue, you agree to hold all parties free and harmless of any claims, demands, or suits for damages arising from the giving of such consent. You also acknowledge that you will be ultimately responsible for the cost of any medical care should the cost of that medical care not be reimbursed by your health insurance provider. Should your child/dependant become ill, or if sent home as deemed necessary by the Church or its agents due to noncompliance with the above basic rules of conduct or list of expectations, you also agree to come and pick them up. Videos, photographs and audio clips may be taken at any of the THRIVE nights, to be used for future promotional material. By registering to attend THRIVE, you are giving permission to use your picture in promotional materials. Please also note that registration fees are not refundable.
